Abstract

The study assessed the EPI services by the RHUs in three districts in Cavite in 2008. It assessed the implementation of the EPI program in terms of the level of support by the Local Government Unit (LGU), the organizational structures implemented, and the processes involved. It also evaluated the impact of immunization on the health status of the population. Furthermore, the study determined the differences in EPI services among the three districts in Cavite in terms of governance, organizational structure, process, and achievement of target indicators. This is a survey research wherein municipal health officers or public health nurses were the key informants interviewed, rural health midwives were observed during immunization days, and clients with 0-6 years old children were interviewed. The EPI performances of the different municipalities from the three districts of Cavite were presented using percentage, means, and standard deviations. EPI performances of the three districts were also compared and differences were determined to be significant using Chi-square test and Analysis of Variance (ANOVA) test. Based on the results, the study concludes that, except for accessibility, the level of support by the LGU in the implementation of the EPI Program in terms of budget/funding, workforce manpower and infrastructure support was not adequate. Also, except for health information system, the level of organizational structures involved in the implementation of EPI services in terms of availability of equipment and supplies, and staff training and development were not adequate. Similarly, in the level of processes involved in the implementation of EPI services, only the domains of immunization schedule and utilization/access were assessed to be adequate. Furthermore, the study concludes that immunization had an impact on the health status of the population in terms of decreasing the incidence rate of measles and reducing mortality due to tetanus in the province of Cavite from 2003 to 2008. Notwithstanding the low FIC, there was no measles mortality in the province from 2003 – 2008. There was high satisfaction among clients of EPI services in the province of Cavite. However, all percentages of Fully Immunized Children fell below the national standard. Finally, the study concludes that the EPI performance among the three districts in Cavite differed in terms of organizational structure specifically in REB staff training among nurses and midwives, and other trainings for midwives. Likewise, target indicators differed significantly. Based on the findings and conclusions, the study recommends that the identified gaps should be addressed by the concerned sectors of the community. The Local Government Units (municipalities/cities) should allocate budget specifically for EPI implementation, establish or strengthen public-private partnerships to improve the immunization coverage, and hire additional nurses and midwives to meet the standard requirement of manpower to population ratio. Moreover, the community should initiate activities that would help generate funds for the construction of new barangay health stations (BHS) or for improvement of existing BHS as this would increase client satisfaction and utilization of health center immunization services. Community residents’ participation in immunization programs should also be encouraged. All RHU staff (RHMs and Nurses) should have regular training regarding EPI services especially on interpersonal skills and health education skills and these should be designed to address the districts’ specific needs/problems when implementing EPI. The Provincial Health Office should closely monitor the practice of active master listing in all municipalities/cities and consider imposing sanctions for noncompliance. All Rural Health Units should have a micro plan to improve the EPI Coverage. Parishes should initiate health education programs on importance of immunization for their parishioners and schools should include EPI in their curricula. Finally, the Department of Health should implement a policy on continuation of logistics support for all devolved municipalities which should include vaccine supplies, equipment and trainings.
